What To Bring
Please bring your drivers license or State Id, Your current MMMP Card even if already expired, any available medical records pertaining to your qualified condition including prescriptions, diagnostic reports, letters from your doctor, or treatment plans.
What To Expect
Checking in: You will be asked to complete brief patient history forms and the required State applications. All forms are provided. If you do not have access to your medical records, you may be asked to sign a medical release form. Be advised that your primary Care physician is NOT notified of your medical marijuana certification unless you give written permission!
Physician Evaluation: Your evaluation by the Doctor is conducted individually in a private office. The Doctor will review provided medical history and the effectiveness of your past medical marijuana treatment. All relevant health information is considered to determine potential medical benefits and re-qualification. Doctor will then re-certify you if you qualify.
Consultation: After completing your Physician Evaluation, you are provided a one on one
individualized consultation by a knowledgeable MMJ Consultant. This includes: Complete State Form Preparation and Mailing Instructions- Full Service Mailing Option provided for those who don’t want the hassle. It is important you understand your rights and responsibilities as a patient or caregiver, best ingestion methods for you to medicate based on your condition, and recommended strains for your condition. Other alternative therapies available as well as immune Building supplements to support optimal overall health. Follow up and Support available.
